Job Overview:

The Project Officer Aged role plays a pivotal part in supporting the strategic objectives of leading support and strengthening person-centred primary health care. This includes developing and coordinating activities related to older people's health, aged care, and palliative care.

Key aspects of this position include implementing The Aged Care On-site Pharmacist Measure (ACOP) in residential aged care homes (RACH) and coordinating the engagement and support for Allied Health sector in Western NSW.

Core Responsibilities:

  • Implementation of ACOP in RACH
  • Coordination of Allied Health sector engagement and support
  • Strengthening person-centred primary health care
  • Development and coordination of activities related to older people's health, aged care, and palliative care

Essential Skills and Qualifications:

  • Experience in project management, including creating and managing project timelines
  • Knowledge of aged care, primary care, community pharmacy, and/or allied health sectors
  • Excellent interpersonal skills with well-developed written communication skills
  • Demonstrated understanding of Aboriginal culture and commitment to cultural awareness and safety
  • Demonstrated high-level interpersonal and communication skills and competence in using information communication technology
  • Capacity to travel within western NSW and hold a current driver's licence
