Job Title: NDIS Support Coordinator

About the Role

The NDIS Support Coordination team is committed to empowering individuals with disabilities to achieve their goals.

As an NDIS Support Coordinator, you will play a vital role in supporting participants in understanding and implementing their NDIS plans, fostering their independence and well-being.

You will collaborate with participants to interpret their NDIS plans, assist them in implementing their plan, and develop individualised support plans aligned with their goals and aspirations.

Key Responsibilities
  • Interpreting participant NDIS plans to ensure they understand their allocated funding and available support options.
  • Implementing participant plans by identifying and connecting them with suitable service providers and community resources.
  • Developing and maintaining individualised support plans aligned with participant goals and aspirations.
  • Advocating for participants to ensure access to culturally safe and appropriate support services that address their individual needs and promote holistic well-being.
Required Skills and Qualifications
  • Relevant tertiary qualification in disability support, social work, health, or a similar field.
  • 2 years' relevant experience in social services or a similar field.
  • Strong communication, interpersonal, and advocacy skills.
  • Ability to network and develop effective working relationships.
  • Passion for empowering individuals with disabilities to achieve their goals.
  • Commitment to providing culturally sensitive and inclusive support.
