Job Overview:
Social Worker RoleThis position entails supporting children in care aged 14 and above as they prepare for adulthood and independence.
Key Responsibilities:- Developing tailored care plans to meet individual needs
- Coordinating services to support education, health, and emotional well-being
- Creating Children Looked After (CLA) Plans, Education, Health and Care (EHC) Plans, and transition plans
- Collaborating with families, carers, and partner agencies to ensure consistent support
- Advocating for the young person's best interests
- Regular reviews, safeguarding, and promoting positive outcomes as they move toward leaving care
- Strong ability to work collaboratively with families, carers, and partner agencies
- Effective communication and advocacy skills
- Capacity to assess individual needs and develop effective care plans
- Experience working with vulnerable populations, particularly children in care
