Job Title: Needs Assessor for Older Adults

We are seeking a skilled and compassionate individual to fulfill the role of Needs Assessor for Older Adults. In this capacity, you will be responsible for assessing the needs of older individuals and developing strategies to support their care.

Key Responsibilities:
  • Conduct in-home assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool (IAT).
  • Determine goals specific to each client and develop suitable strategies and actions.
  • Generate codes for services for referral to age care and non-aged care services.
  • Facilitate referrals and create Support Plans.
Requirements:
  • Well-developed written, verbal, and interpersonal communication skills.
  • Proven computer proficiency with capacity to learn and utilize electronic reporting systems.
  • Able to work independently as well as effectively within a collaborative team environment.
  • Equivalent experience working with older adults and/or relevant qualifications such as Certificate III in Home Care or Aged Care or Allied Health Assistance.
