Care Professional for Older Adults

This role offers the opportunity to make a real difference in the lives of older adults living independently in the community.

You will work as part of a multidisciplinary team, providing early intervention, health education, and recovery-focused support within clients' homes and community settings.

The focus is on delivering safe and quality care to clients with specific needs and/or complex needs, such as wound care and palliative care.

  • Responsibilities include:
    • Providing direct clinical nursing care to our clients in their homes, in line with best practice nursing procedures.
    • Conducting clinical assessments, dressings, medication assessment, and catheter changing.
    • Developing a holistic nursing care plan in collaboration with the client, carer, Care Advisor, and Allied Health professionals where relevant.
    • Ensuring all documentation is accurate and completed in a professional and timely manner.

To be successful in this role, you will need:

  • A genuine passion for providing high-quality care to the elderly.
  • Current AHPRA registration following completion of a Bachelor of Nursing or similar.
  • Minimum 3 years post-graduate nursing experience.
  • Experience in clinical assessments.
  • Initiative and the ability to work independently.
