**About the Role:**
We are seeking a highly skilled Registered Nurse to join our team in providing high-quality care to older people in inpatient settings. The successful candidate will be responsible for identifying and coordinating community or residential care services on discharge from hospital.
The primary focus of this role is to facilitate timely discharges by assisting older people and their carers/families in accessing appropriate services.
This position plays a critical role in improving the assessment, care planning, and discharge planning of older people with complex care needs in the inpatient setting.
Key Responsibilities:- Assist older people and their carer/families to access appropriate services to facilitate timely discharges from hospital
- Improve the assessment, care planning, and discharge planning of older people with complex care needs in the inpatient setting
- Motivated and experienced individual who possesses effective communication skills through various means
- Ability to utilise a range of communication strategies to negotiate resolutions and manage conflict
- Excellent time management and organisation skills for administration and clinical duties
You will be working alongside a dedicated team of Allied Health, Medical staff, Registered Nurses, Endorsed Enrolled Nurses, and Assistants in Nursing in supporting excellent care to patients in collaboration with patients, families, and carers.
