Outreach Specialist

This role presents a unique opportunity to make a meaningful difference in the lives of individuals experiencing chronic homelessness. As an Outreach Specialist, you will work intensively with a small caseload, delivering outreach-based case management and service coordination, and empowering clients to access essential resources.

About the Role

Located in Geelong VIC, this permanent full-time position offers a chance to drive lasting stability and dignity for vulnerable members of our community. You will build deep relationships through assertive and persistent engagement, and collaborate with professionals to provide culturally safe support.

Key Responsibilities
  • Work closely with individuals experiencing complex needs, including homelessness, mental health, substance use, and family violence
  • Develop and implement tailored plans to address client goals and objectives
  • Build trust and rapport through regular outreach and communication
  • Collaborate with multidisciplinary teams to ensure comprehensive support
  • Capture and analyze data to inform program development and improvement
Requirements
  • A relevant tertiary qualification in Social Work, Psychology, Community Services or related field
  • Demonstrated experience working with vulnerable populations and addressing complex needs
  • Proven ability to work independently and collaboratively within a team environment
  • Strong communication and interpersonal skills, with a focus on empathy and understanding
  • Current Employee Working with Children's Check and VIC Driver's Licence
