Make a meaningful difference in the lives of community aged care clients as a Case Manager with our Home Care Package service.

**Job Description:**

We are seeking an experienced and passionate professional to oversee the case management of our Community Aged Care clients. The successful candidate will be responsible for assessing, coordinating and reviewing client care needs within their budget.

The ideal candidate will have a genuine passion for delivering flexible and responsive services tailored to an individual's needs and interests, while also providing advocacy and referral services to clients.

**Key Responsibilities:**

  • Acknowledge and address client care needs within budgetary constraints.
  • Coordinate and manage client care plans, including referrals to external services.
  • Monitor and review client outcomes, making adjustments as necessary to ensure optimal results.

**Required Skills and Qualifications:**

To be successful in this role, you will need:

  • Demonstrated experience in case management, including budgets.
  • Experience with Home Care Packages within the Aged Care industry or transferable case management skills in another field.
  • Highly developed planning, influence and negotiation skills.
  • Demonstrated experience in managing the contractual requirements of government programs to ensure compliance, regulatory standards and organisational policy and procedure.
  • High degree of computer literacy with an ability to learn new software platforms.
  • Registered and insured motor vehicle and current unrestricted drivers' licence.
