About the Role

Home care package coordinators are responsible for supporting clients who receive home care packages. They coordinate and schedule care delivery, providing on-call community care services as needed.

Key Responsibilities:

  • Communicate with potential new clients to offer services and demonstrate benefits
  • Complete initial telephone assessments to understand client needs and goals
  • Ensure accurate documentation and data entry for effective service scheduling and billing
  • Manage complaints and incidents effectively, escalating when necessary
  • Complete client reviews by telephone for changes in need or program intervals
  • Refer clients for ACAT assessment for Home Care Package eligibility
  • Encourage client feedback

Requirements:

  • Comprehensive knowledge of ageing issues
  • Knowledge of Aged Care Quality Standards
  • Ability to undertake high-standard case management
  • Excellent presentation, communication, and documentation skills
  • Strong organisational capabilities and problem-solving skills
  • Computer literacy and competence in app use
  • Self-motivation with capacity to contribute significantly

Desirable Qualifications:

  • Qualification in aged care or community services or Certificate IV in Service Coordination
  • Relevant experience in case management
  • Current driver's license
  • Access to a registered and reliable vehicle with comprehensive car insurance
