Job Title: Community Case Manager

At our organization, we are seeking a skilled Community Case Manager to join our team.

This role is ideal for an individual who has experience in community-based case management and is passionate about supporting elderly people to live independently.

The successful candidate will have excellent communication skills, both verbal and written, as well as strong problem-solving abilities and critical thinking skills.

About the Role

This position involves managing a portfolio of community-based elderly customers, assessing their needs and developing personalized care plans.

The successful candidate will also be responsible for signing up new customers, proactively managing existing ones, and collaborating with nurses and allied health professionals.

Key Responsibilities:

  • Manage a portfolio of community-based elderly customers

Requirements:

  • Previous experience in community-based case management
  • Experience with Home Care Packages desirable
  • Current driver's license and reliable vehicle insurance
  • Strong communication skills, both verbal and written
  • Demonstrated empathy and compassion
  • Excellent problem-solving abilities and critical thinking skills
  • Good organizational and time management skills
  • Cultural competence and ability to work with diverse populations
  • Advocacy skills to ensure clients' needs are met
  • Collaboration and teamwork skills to work effectively with interdisciplinary teams
