Clinical Care Specialist

We are seeking an experienced and skilled Clinical Care Specialist to join our healthcare team in the Hobart region.
The successful candidate will be responsible for delivering high-quality patient care services across a variety of settings, including Aged Care Facilities throughout Hobart, TAS.
Key responsibilities include:
- Providing direct patient care and managing patient needs with compassion and empathy
- Collaborating with multidisciplinary teams to develop and implement individualized care plans
- Maintaining accurate records and reporting any concerns or issues promptly
Requirements:
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- Relevant nursing degree/qualification and/or relevant work experience
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations
- NDIS/WWVP clearance
