Wellbeing Manager

We are seeking a highly skilled and experienced Wellbeing Manager to join our team at Feros Care. As a Wellbeing Manager, you will play a critical role in delivering high-quality care and support to our clients.
Key Responsibilities- Conduct comprehensive assessments and develop individualized care plans that promote client independence and well-being.
- Build and maintain strong relationships with clients, families, and internal stakeholders to ensure seamless care coordination.
- Collaborate with multidisciplinary teams to provide holistic care and support services.
- Develop and maintain knowledge of relevant policies, procedures, and legislation to ensure compliance and best practice.
- Participate in quality improvement initiatives and contribute to the development of new programs and services.
- Tertiary qualifications in Nursing or Allied Health (with relevant registration).
- Previous case management experience, particularly in community and aged care settings.
- Experience conducting HPC/STRC assessments and applying goal-oriented, outcome-focused approaches.
- Demonstrated ability to develop and maintain strong networks with internal and external stakeholders.
- Current driver's license and willingness to travel throughout the region.
- COVID-19 vaccination.
