About the RoleThis role involves conducting assessments in various settings, including home environments, hospitals and community facilities. You will work collaboratively with clients, their families and other healthcare professionals to develop personalized support plans that focus on client goals and rehabilitation strategies.
- Conduct detailed assessments of individuals requiring care and support
- Identify resources, strengths and challenges of individuals and develop coordinated support plans
- Work collaboratively with clients, their families and healthcare professionals to achieve client goals and enhance quality of life
The ideal candidate will possess exceptional communication skills, attention to detail, and advanced clinical evaluation skills. Relevant qualifications, experience in nursing or allied health, and current registration with relevant professional bodies are also essential.
