Mental Health Nurse Specialist

Seeking an experienced Registered Nurse to lead our multi-disciplinary team in providing comprehensive care and support for older adults with complex mental health needs.
About the Role- Collaborate with healthcare professionals to develop and implement evidence-based care plans that address the unique needs of each consumer.
- Conduct thorough mental health assessments, identifying areas for improvement and developing targeted interventions.
- Foster strong relationships with consumers, carers, and community services to ensure seamless care coordination and support.
- Lead multidisciplinary teams in assessing and managing complex mental health issues among older adults.
- Develop and implement individualized care plans that prioritize consumer outcomes and promote recovery.
- Cultivate partnerships with external agencies to enhance access to resources and services for consumers.
- Registered Nurse with at least 5 years of experience in mental health services, particularly with older adults.
- Postgraduate qualification in mental health or equivalent experience.
- Demonstrated expertise in care coordination, assessment, and intervention development.
- Strong communication and interpersonal skills.
- Able to work independently and as part of a team.
- Experience working with consumers and carers with complex mental health needs.
