Life Skills Coach

Reporting to the After Hours Client Services Coordinator, this is a permanent full-time position, based in Shepparton, VIC. Salary and conditions are in accordance with the SCHADS Award Level 5.
Key Responsibilities:
\u2022
Undertake case management with clients utilising case management frameworks and tools as per TSA policy and procedure
\u2022
Maintain partnership arrangements, building service relations and inter-agency relationships
\u2022
Ensure accurate, up to date client records and data are maintained for all clients in accordance with TSA standards using TSA information systems
\u2022
Participate in client feedback sessions and completion of outcome reports as required by the agency and funding bodies
\u2022
Identify risks to clients, staff, and the organisation and reporting these to management as soon as possible
Requirements:
\u2022
Tertiary qualification in social work, welfare, community development or related field
\u2022
Relevant experience in a social service environment preferably the youth sector
\u2022
A current and valid Working with Children\u2019s Check
\u2022
A current and valid Victorian Driver\u2019s Licence
