
Supporting Vulnerable Communities as a Homelessness Case Manager

We are seeking a dedicated and compassionate individual to fill the role of Homelessness Case Manager. As a key member of our team, you will play a crucial part in supporting individuals who are homeless or at risk of homelessness.
The ideal candidate will possess excellent communication skills, be able to build strong relationships with participants and stakeholders, and demonstrate a passion for delivering high-quality case management services.
Key Responsibilities:
- Develop, implement, and review case management plans that meet the unique needs of each participant.
- Provide personalized support, guidance, and advocacy to ensure participants receive the best possible outcomes.
- Conduct thorough assessments of participant needs, including risk assessments and safety planning as required.
- Maintain accurate records and reports, ensuring compliance with organizational policies and procedures.
To be successful in this role, you will need to possess the following skills and qualifications:
- Certificate IV qualification in Community Services or a related field.
- Proven ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple deadlines.
- Excellent communication and interpersonal skills, with the ability to build rapport with diverse groups of people.
- Able to obtain a Blue Card (Working with Children Check) and Yellow Card (Disability Services Positive Notice).
