Support Coordination Specialist

  • Deliver quality support coordination for individuals with disabilities.
  • Coordinate services and manage complex multi-disciplinary teams.
  • Ensure timely delivery of high-quality services, including mandatory reporting to the relevant authorities.
  • Provide expert advice and consultation to individuals, their families, and stakeholders in response to challenging situations.
  • Develop action plans to resolve complexity and provide ongoing support and guidance.

Key Responsibilities:

  • Perform specialist support coordination to assist participants in implementing their NDIS plan.
  • Coordinate services in accordance with participants' NDIS plans.
  • Manage complex relationships with stakeholders, including external partners and suppliers.
  • Provide leadership and support to other support coordinators in relation to support coordination delivery.
  • Act as a spokesperson and liaison for Immunity Care, effectively managing relations with external stakeholders, suppliers, and the NDIS.

Requirements:

  • At least 2 years of experience in a previous role.
  • Expert communication (both written and oral) skills and ability to manage complex relationships.
  • Flexible and able to work effectively within a changing environment.
  • Great level of understanding of the NDIS system and price guide.
  • A tertiary qualification in one of the below disciplines: Psychology, Occupational Therapy, Social Work, Speech Pathology, Other highly trained allied health profession, Developmental education, or Social and/or health science.
