Health Access and Equity Coordinator

Job Description:

The position of Health Access and Equity Coordinator will play a pivotal role in supporting the planning and response to health system reform, strategic priorities, and identified gaps in our community.

  • Coordinate the successful implementation of The PRIDE Co-Design Project by engaging primary healthcare clinical sites and providing quality improvement coaching.
  • Contribute to existing and emerging health access and equity initiatives with PHN teams, key partners, and sector stakeholders.

This is an entry-level position that requires strong project management experience in health and a passion for improving health outcomes.
