Care Coordinator for Adolescent Healthcare

We have an exciting opportunity for a Clinical Nurse to join our Paediatric Medicine team. Your role will involve:
- Delivering specialist clinical care across Adolescent Medicine Outpatient Services, including Eating Disorders, Gender Diversity, Adolescent Transition, and Complex Medical Clinics, with a focus on patient-centred care.
- Collaborating in multidisciplinary teams to assess, plan, implement, and evaluate therapeutic interventions and support programs for young people and their families.
- Leading and supporting clinical education by mentoring junior staff, facilitating student learning, and contributing to a positive team culture.
- Driving service improvement by contributing to research, clinical audits, and the development of protocols, while supporting change and innovation across adolescent health services.
To be successful, you will need:
- A Registered Nurse qualification with a current practicing certificate.
- Strong communication, problem-solving, and teamwork skills, with the ability to work independently and manage competing priorities.
- At least 3 years' post-registration experience, with demonstrated competence in adolescent healthcare.
- Proven ability to mentor junior staff and support student learning within a multidisciplinary team.
- Sound knowledge of healthcare legislation, quality improvement systems, and current clinical best practices.
