Renal Care Coordinator

Our organization seeks a skilled RN Care Manager to join our Transitional Care Management team. This role requires a compassionate and organized individual who can effectively coordinate care for patients with complex Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD).
The RN Care Manager will serve as a specialized clinical resource, working closely with patients, families, and interdisciplinary care teams to ensure safe, effective, and timely transitions between care settings.
This position plays a critical role in minimizing readmissions and improving patient outcomes across the care continuum. The ideal candidate will have excellent communication skills, be able to work independently, and have a strong understanding of case management programs.
Key Responsibilities:- Craft personalized and thorough care plans addressing the clinical and non-clinical needs of each patient.
- Conduct regular assessments virtually to detect undisclosed medical or social issues and changes in the home environment that could pose risks.
- Perform proactive outreach to patients and providers following TCM guidelines to evaluate status and address needs.
