Job Description

A Kidney Care Coordinator ensures the best possible outcomes for individuals with kidney disease by fostering seamless transitions between care settings.

This role requires a comprehensive understanding of transitional care models and strong collaboration with patients, families, and interdisciplinary teams to craft personalized care plans addressing both clinical and non-clinical needs.

Key Responsibilities:
  • Craft thorough care plans that address the unique requirements of each patient, in collaboration with the care team.
  • Conduct regular assessments virtually to identify potential risks and promptly refer matters to relevant channels for resolution.
  • Perform proactive outreach to patients and providers, adhering to established guidelines, to evaluate status and address needs.
  • Manage a diverse patient panel efficiently, ensuring accurate evaluations of individual needs while maintaining adherence to NCQA standards for case management.
  • Communicate updates to the care team and family members, identifying gaps in patient understanding of their health status and delivering tailored education and resources to promote informed decision-making.
