  • Communications with the care team and family members are crucial, identifying gaps in patient understanding of their health status and delivering tailored education and resources to promote informed decision-making.
  • They act as liaisons with dialysis facilities, conveying care plan updates to facilitate smooth transitions in care, and maintain accurate program software records.
  • RN care managers leverage data platforms to identify trends, anomalies, and areas requiring focus.
  • Providing culturally sensitive care is essential to meet the diverse needs of patients, fostering collaboration and communication among all clinical team members to enhance care provision.

Key Requirements:

  • Associate's degree from an accredited school of nursing.
  • Current Registered Nurse (RN) license in the state of practice.
  • Minimum 3 years combined experience, education, or certification in a related field.
  • Current BLS or CPR Certification required.
