About the Role

We're seeking a dedicated Kidney Care Specialist to join our team. As a key member of our care delivery model, you'll be responsible for developing personalized care plans that address the clinical and non-clinical needs of each patient.

Key responsibilities include conducting regular assessments onsite and virtually to identify potential risks and issues, initiating proactive outreach to patients and clinics as outlined in program guidelines, and communicating updates to the care team and family members.

You'll work closely with patients and their families to foster engagement and promote informed decision-making, ensuring seamless coordination of care across multiple settings.

Responsibilities:

  • Craft thorough care plans addressing patient needs
  • Conduct regular assessments onsite and virtually to detect undisclosed medical or social issues
  • Initiate proactive outreach to patients and/or clinics
  • Communicate updates to the care team and family members

About You

We're looking for individuals with experience developing strong patient relationships, demonstrated proficiency in documenting and navigating electronic medical records (EMR) and care plan management, and a passion for delivering exceptional care.
