Job Description:

We are seeking a skilled Care Manager to join our team. As a Care Manager, you will play a vital role in ensuring the safe and effective transition of patients with complex Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD) between care settings.

Key Responsibilities:
  • Develop personalized care plans: Collaborate with interdisciplinary care teams to create comprehensive care plans that address the unique needs of each patient.
  • Conduct regular assessments: Virtually assess patients to detect potential issues and risks, referring them to appropriate channels for resolution.
  • Provide proactive outreach: Conduct regular outreach to patients and providers to evaluate status and address needs in accordance with TCM guidelines.
  • Manage patient panel: Swiftly and accurately evaluate individual patient needs while adhering to NCQA standards for case management.
  • Communicate updates: Effectively communicate updates to the care team and family members, identifying gaps in patient understanding and delivering tailored education and resources.
  • Liaise with dialysis facilities: Convey care plan updates to facilitate smooth transitions in care and maintain accurate program software records.
