Job Opportunity:

The Housing and Wellbeing Case Manager role is a fantastic opportunity for individuals who are passionate about empowering people to overcome housing and wellbeing challenges. The successful candidate will work closely with clients to develop and implement personalized care plans, providing high-quality support and interventions that meet their unique needs.

About the Role

This role requires a professional who is able to provide outreach-based services, engaging with clients in crisis accommodation, street outreach, and refuges. You will translate initial engagement into a strong ongoing partnership, building trust and rapport with clients.

  • Provide supports and interventions based on the program model of care.
  • Develop and maintain high-quality case management practices and record keeping that adhere to operational requirements.
  • Engage clients through a positive and assertive outreach approach.
  • Facilitate client-led assessments and Care Plans, identifying client goals and working collaboratively with clients to achieve them.
Requirements

To be successful in this role, you will possess:

  • A current Cert IV in Social Work, Human Services, or a related field, with at least 12-months experience in a case management role.
  • Alternatively, a current graduate university bachelor's degree in social work, human services, or a related field, with at least a 10-week-full-time placement in social services.
  • Demonstrated ability to work in a performance-driven environment, meeting individual weekly, monthly, and quarterly performance targets.
  • Ability to build client agency, self-efficacy, and facilitate change through client empowerment.
