Empowering Lives: Housing and Wellbeing Case Manager Role

We are seeking a highly motivated and compassionate Housing and Wellbeing Case Manager to join our team. The successful candidate will work with individuals and families in partnership to empower them to overcome their identified housing and wellbeing needs.
The role is heavily outreach-based, engaging individuals and families in crisis accommodation, street outreach and refuges, and translating initial engagement into a strong ongoing partnership.
Key Responsibilities:- Provide high-quality case management practices and record-keeping that adhere to operational requirements.
- Facilitate client-led assessments and Care Plans to identify clients' goals, including the entire family where appropriate.
- Provide role-specific targeted interventions and family group meetings in achieving client goals.
- A current Certificate IV in Social Work, Human Services, or related field, with at least 12 months of experience in a case management role.
- Demonstrated ability to build client agency, self-efficacy, and facilitate change through client empowerment.
- Ability to work in a performance-driven environment to meet individual weekly, monthly, and quarterly performance targets.
