Care Coordinator

We are seeking passionate individuals to join our dedicated care team in a full-time capacity.

  • Possess in-depth knowledge and comprehension of the Aged Care Quality Standards, Aged Care Legislation, and related principles.
  • Exhibit a strong commitment to providing person-centred support to clients.
  • Possess knowledge of the Aged Care sector, including Home Care Packages (HCP) and Commonwealth Home Support Programme (CHSP).
  • Deliver person-centred care coordination to consumers and assist in the day-to-day operations of our organisation.
  • Undertake initial intake assessments.
  • Maintain accurate administration, including updating reports and government portals.
  • Ensure compliance with relevant regulations and best practices.
  • Perform additional responsibilities as outlined in the position description.
  • Familiarity with the NDIS Quality Standards is desirable.

Key requirements:

  • Diploma or Cert IV qualification in Health, Community Services, Welfare, or Social Science.
  • Working with Children Check.
  • Current driver's license and access to a comprehensively insured vehicle.
  • Proven experience in community care operations within a healthcare setting.
  • Ability to multitask and prioritise workload effectively with effective communication skills.
