Nurse Advocate

Providing high-quality nursing services is a critical component of our organisation's commitment to exceptional care. This role involves working with community clients in Port Stephens, where you will utilise your skills and experience to deliver tailored care plans that meet the unique needs of each individual.
Responsibilities- Assess client requirements and develop comprehensive care plans
- Monitor vital signs and respond promptly to changes in health status
- Administer treatments such as injections, wound care and medication management
- Support chronic disease management, including diabetes, dementia and continence care
- Provide palliative care and support for patients with life-limiting conditions
- Current registration with AHPRA (Australian Health Practitioner Regulation Agency)
- Sound knowledge of current clinical practice and aged care standards
- Ability to work effectively with multidisciplinary teams
- Excellent verbal and written communication skills
- Strong commitment to person-centred care and empathy for the needs of the elderly
