Senior Patient Care Specialist

As a skilled nurse, you will be responsible for providing high-quality patient care in a fast-paced ambulatory care environment.
">- Conduct comprehensive nursing assessments of patients to identify their needs and develop individualized care plans.
- Ensure accurate and timely documentation of patient care and progress in medical records, communicating effectively with the healthcare team.
- Collaborate with multidisciplinary teams to plan and implement appropriate patient care, prioritizing interventions to achieve optimal outcomes.
- Identify and implement discharge-planning requirements to ensure smooth transitions of care.
- Evaluate patient progress and revise nursing interventions as needed to ensure effective care.
To be successful in this role, you will require:
- A current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
- A minimum of 12 months recent experience in cancer nursing or an acute care setting.
- A Graduate Certificate in Cancer Nursing or similar qualification.
- Demonstrated ability to work effectively in a dynamic team environment and adapt to changing demands.
- Excellent communication, organisational, and interpersonal skills.
- A commitment to ongoing professional development and maintaining currency in your practice.
You will have the opportunity to work across multiple departments, including Medical Centre, Radiation Oncology, and Day Treatment Centre, providing you with a diverse and challenging work environment.
