Community Rehabilitation Therapist – Adult Services

We are seeking a skilled Occupational Therapist to provide comprehensive assessments, interventions, and therapies to adults under the NDIS. As a key member of our multidisciplinary team, you will work partly clinic-based and out in the community to identify individualised goals, interventions, and therapies for adults.
Key Responsibilities:- Provide high-quality and person-centered OT services including assessment, development of comprehensive goal-oriented treatment and care plans, home assessment, equipment prescription, and report writing.
- Demonstrate a range of intervention and therapy strategies in line with goals.
- Complete detailed reports including home modification reports, equipment applications.
- Communicate effectively with consumers and relevant internal and external stakeholders.
- Conduct internal and external referrals to other members of the multidisciplinary team and external funding providers.
To be successful in this role, you will have a relevant qualification in Occupational Therapy and AHPRA registration. You should also possess excellent communication and interpersonal skills, as well as the ability to prepare professional reports and recommendations.
A Victorian drivers license and current Working With Children Check are also essential requirements for this position.
