Housing Support Professional

  • Undertake case management, including intake, assessment, case planning, coordination, and key worker roles
  • Establish and maintain action plans and outcome star plans for adults and families
  • Collaborate with local housing offices, other support providers, and stakeholders to advocate for clients and connect them to essential services
