About the RoleAs part of our front-line team, you will provide vital support to adults experiencing or at risk of homelessness.
You will undertake case management, crisis intervention, and case planning to help individuals and families access safe housing, healthcare, and critical social services.
No two days are alike – you will be out in the community building connections and working collaboratively with your team to drive lasting change.
Your Key Responsibilities- Undertake thorough case assessments, develop action plans, and establish outcome-based goals for adults and families.
- Work closely with local services, government agencies, and community organizations to deliver comprehensive support.
- A minimum qualification of a sector-recognised Diploma in Youth Work, Social, or Welfare Work or equivalent.
- A valid driver's license.
