Key Responsibilities:

Work effectively with patients, families, carers and other stakeholders to identify their needs and develop strategies to support them.

Requirements:

Clinical experience as a social worker

Postgraduate qualifications in social work or a related field

Ability to work effectively in a dynamic team environment
