Job Summary

  • We are seeking a skilled Social Worker to join our Adult Combined Locality Team in Knowsley. The ideal candidate will have experience in delivering care assessments and strengths-based support planning, as well as commissioning appropriate care services.

Key Responsibilities:

  • Care Act Assessments: Deliver thorough Care Act assessments to identify individual needs and develop effective support plans.
  • Care Service Commissioning: Commission suitable care and support services that meet the needs of service users.
  • Safeguarding Enquiries: Conduct safeguarding enquiries, Mental Capacity Assessments, and make informed Best Interest decisions.
  • Risk Management: Manage and assess risk, attend Multi-Agency Risk Assessment Conferences (MARAC), and contribute to coordinated safety planning.
  • Continuing Healthcare Processes: Participate in Continuing Healthcare processes, including completion of Decision Support Tools (DSTs).

Requirements:

  • Registration: Registration with the Social Work England (SWE) register is essential.
  • Eligibility: Eligibility to work in the UK is required.
  • Driving License: A full UK driving license is necessary for travel between locations.
  • Post-Qualification Experience: Post-qualified experience in social work is desirable.
  • Educational Qualifications: A degree level or equivalent qualification in Social Work is required.
