Position Overview

We are seeking a highly skilled Clinical Nurse Consultant to provide expert clinical guidance and leadership in the Aged Care Rapid Response Team. The role involves coordinating comprehensive hospital avoidance strategies to deliver optimal care for people aged 70 and over within Residential Aged Care Facilities (RACFs) and the community.

The successful candidate will facilitate timely access to diagnostic, assessment, and treatment services with the aim of avoiding hospital presentation or returning individuals to their residences with supportive strategies in place.

Main Responsibilities
  • Provide authoritative advice and recommendations on all aspects of nursing practice, ensuring the safe and appropriate use of nursing interventions.
  • Develop and implement policy, guide practice development, and engage in education and research activities that enhance patient outcomes.
  • Offer clinical supervision to nursing staff, acting as a role model and providing regular nursing care.
Essential Skills and Qualifications

To succeed in this position, you will require:

  • Advanced nursing qualifications and experience in gerontology or a related field.
  • Proven expertise in developing and implementing comprehensive care plans.
  • Excellent communication and interpersonal skills, with the ability to work effectively with multidisciplinary teams.
