Job Title: Complex Case Manager Admin

About the Role:

We are seeking a highly skilled and experienced Complex Case Manager to join our team at Healthcare Australia. As a key member of our multidisciplinary team, you will be responsible for providing guidance, training, and supervision to ensure effective service delivery.

Key Responsibilities:

  • Manage patient files, ensuring compliance with policies and procedures, and communicate with external agencies.
  • Ensure that policies and procedures are being followed, including safety protocols, guidelines, and facility regulations.
  • Coordinate communication between different departments and external agencies.
  • May supervise administrative staff and ensure operational efficiency in terms of case handling, scheduling, and facility management.
  • Responsible for data collection and reporting, tracking patient statuses, and creating reports for internal or external review.

Qualifications:

  • Bachelor's degree in Social Work, Nursing, Healthcare Management, or a related field.
  • Experience in case management is essential, with a minimum 5 years in similar environments, such as social services, mental health, or correctional settings.
  • Ability to work collaboratively with a diverse range of stakeholders.
  • Strong organisational and administrative skills to manage complex cases.
  • High level of accuracy in record-keeping and reporting.
  • Ability to adapt to changing circumstances and respond effectively to emerging health risks or issues.
  • Knowledge of Australia's public health system and relevant healthcare regulations.
