Senior Case Manager

At Healthcare Australia, we are seeking a highly skilled and dedicated Complex Case Manager Resident to join our team. As a key member of our team, you will play a vital role in providing comprehensive support to patients and ensuring effective coordination of healthcare and wellbeing services.
Key Responsibilities:- Develop and implement case management processes to monitor individual cases and provide direction to resolve complex issues.
- Collaborate with healthcare providers and stakeholders to facilitate communication between patients and their healthcare options.
- Identify and mitigate health risks, ensuring alignment with public health standards through regular reviews of health risk management strategies.
- Coordinate mental health promotion activities and monitor their effectiveness, adjusting programs as necessary to meet patient needs.
- Bachelor's degree in Social Work, Nursing, Healthcare Management, or a related field.
- Minimum of 5 years of experience in case management, healthcare coordination, or a related field.
- Ability to work collaboratively with a diverse range of stakeholders.
- Strong analytical skills to identify and address health risks and issues.
- High level of accuracy in record-keeping and reporting.
- Ability to adapt to changing circumstances and respond effectively to emerging health risks or issues.
- Knowledge of Australia's public health system and relevant healthcare regulations.
