Job Opportunity: Care Coordination Specialist

This role presents a chance to make a meaningful impact on the lives of our community.

As a Care Coordination Specialist, you will provide high-quality care coordination services for clients with chronic diseases and complex needs. Your primary responsibility will be to work effectively with Indigenous outreach workers to engage with clients, arrange necessary services outlined in their management plans, and ensure transport arrangements are in place to facilitate their attendance at appointments.

You will also involve clients' family/carers as appropriate, assist them in adhering to treatment programs, and develop chronic condition self-management skills. Additionally, you will provide clinical care consistent with the requirements of this role.

Maintaining accurate client medical records, employing a holistic, culturally sensitive approach to the client, and effective communication are key aspects of this position.

We seek an individual with experience providing one-on-one support to clients, ability to collect data, and write reports, as well as understanding of the issues and barriers impacting the safety and wellbeing of Aboriginal and Torres Strait Islander people.

A tertiary qualification in community services, health, or a relevant professional field is preferred but not essential. High level of personal resilience and flexibility to handle challenging situations are required.
