Senior Community Care Coordinator

We are seeking a Wellbeing Manager to coordinate care delivery and make a daily impact in the community. The successful candidate will be responsible for managing care plans, providing nursing care, and ensuring holistic person-centred care at home.
Job Responsibilities:- Coordinate tailored Home Care Package (HCP) plans to ensure comprehensive care at home.
- Provide nursing care one day per week, including assessments, treatments, and care planning.
- MUST HAVE Current Registered Nurse (AHPRA) with hands-on experience and passion for supporting older adults.
- Strong case management skills, HCP assessment experience, and goal-oriented approach.
- Excellent communication and stakeholder engagement, valid driver's licence, and willingness to travel locally.
