Chronic Disease Care Coordinator

Delivering high-quality care coordination for clients with chronic diseases and complex needs is a rewarding career path. We are seeking an experienced professional to work effectively with Indigenous outreach workers, engage with clients, and arrange services outlined in the client management plan.
The ideal candidate will have expertise in providing one-on-one support to clients, collecting data, writing reports, and client notes. They must also possess a deep understanding of the issues and barriers impacting the safety and wellbeing of Aboriginal and Torres Strait Islander people.
