Support Specialist for At-Risk Individuals

We are seeking a skilled and compassionate Homelessness Outreach Specialist to join our team. As part of this role, you will be responsible for providing support and guidance to individuals experiencing homelessness.
In this position, you will develop strong relationships with clients, working closely with them to understand their needs and goals. You will create personalized case plans, identifying the resources and services required to help clients achieve stability and independence.
You will work collaboratively with internal and external partners, coordinating access to essential services such as housing, healthcare, mental health support, and social connections. Accurate and timely documentation is crucial in this role, ensuring seamless continuity of care for our clients.
- Main Responsibilities:
- Build trust-based relationships with clients through persistent and respectful engagement techniques.
- Develop and implement individualized case plans, aligned with client goals, strengths, and immediate needs.
- Facilitate access to essential services, including housing, health services, mental health support, drug & alcohol programs, legal services, and community connections.
- Coordinate case conferences and planning client exits with continuity of care.
- Ensure accurate and timely documentation, data entry, and reporting.
