Senior Healthcare Leader

We are seeking a highly skilled and experienced Senior Healthcare Leader to lead our multidisciplinary team delivering life-changing care to older adults in their own homes.

About the Role

The successful candidate will be responsible for leading a passionate multidisciplinary team providing short-term case management to prevent hospital admissions through the delivery of care in the home environment.

  • Apply clinical expertise to assess clients, select and implement different therapeutic interventions and/or support programs, and evaluate patient/client progress.
  • Link clients and carers with relevant community resources and services.
  • Communicate with the client's family, carers, and other service providers.
Requirements

To be considered for this role, you must have:

  • Registered Nurse with at least 3 years, full-time equivalent, post-registration experience or an Allied Health professional holding an appropriate degree level qualification and membership (or eligibility for membership) with the relevant professional association, and or registration with the relevant Board.
  • Experience in the supervision of students, and less experienced staff.
  • Effective leadership skills, including highly developed skills in communication, problem-solving, conflict resolution, and negotiation skills.
  • Clinical experience as a senior practitioner within a subacute setting, including clinical experience with complex patients.
