Support Coordinator Role

We are seeking an experienced and passionate Support Coordinator to join our team. As a Support Coordinator, you will work closely with NDIS participants to help them achieve their goals and live independently.

  • Coordinate the day-to-day operation of NDIS Support Coordination Services.
  • Contribute towards participant assessment care planning, respond to referrals and reviews in line with our policies and procedures.
  • Advocate internally and externally to ensure systems and services protect the rights of participants and give them the greatest amount of choice and control over decision-making processes.

To be successful in this role, you will need:

  • A minimum Certificate III in Disability Services or a relevant qualification.
  • Experience working with people living with a disability.
  • Knowledge of the NDIS Operational Guidelines.
  • Ability to liaise with other health and government systems.
