We are seeking a skilled Disability Support Worker to join our organisation. This is an incredible opportunity to work with a team that empowers vulnerable people and individuals living with disability.
Our organisation provides in-home and community care support services to disabled people of all ages, focusing on their needs and aspirations. We strive to give them the best life they can achieve within our capability.
Key Responsibilities
- Deliver person-centred care to help customers achieve their goals and aspirations
- Contribute information for the development of customer support plans
- Provide support with personal care, living skills, transport, social activities, medication and other needs as required
- Keep accurate and up-to-date records of customer interactions
- Develop and maintain professional relationships and networks with customers, families, caregivers and key stakeholders
- Identify innovative ways to enhance the customer journey with support from our team
Required Skills and Qualifications
- Cert 3 or 4 in disability, individual support, aged care or nursing
- Experience in disability, aged care or nursing sector
Personal Attributes
- Ability to be proactive, take initiative and eager to learn
- Ability to work in a fast-paced environment
- Strong communication, problem-solving and conflict resolution skills
- Excellent time management, multi-tasking and organisational skills
- Ability to work under pressure and time constraints with flexibility
- Excellent attention to detail
