Support Services Professional

We are seeking a dedicated Support Services Professional to join our team. As a key member of our support team, you will play a crucial role in empowering individuals with disabilities to live fulfilling and independent lives.

Job Overview:

As a Support Services Professional, you will provide direct support and assistance to individuals with disabilities, helping them to achieve their goals and participate fully in their communities.

Key Responsibilities:

  • Provide one-on-one support to individuals with disabilities in their homes, community settings, and recreational activities.
  • Assist clients with daily living tasks, including personal care, meal preparation, medication management, and household chores.
  • Support clients to develop independent living skills, such as budgeting, meal planning, and transportation.
  • Facilitate community participation and social inclusion by accompanying clients to appointments, outings, and social events.
  • Implement and monitor individualized support plans, ensuring that goals and outcomes are achieved in a person-centered manner.
  • Maintain accurate and timely documentation of client progress, activities, and incidents as required.
  • Communicate effectively with clients, families, and other stakeholders to ensure that support needs are met and preferences are respected.
  • Collaborate with multidisciplinary teams, including therapists, case managers, and healthcare professionals, to coordinate services and achieve positive outcomes for clients.

Qualifications and Skills:

  • A certificate in aged care or home and community care is mandatory.
  • Previous experience working in disability support, aged care, or a similar role is desirable but not essential.
  • Excellent communication and interpersonal skills, with the ability to build rapport and establish trusting relationships with clients.
  • Compassionate, patient, and empathetic approach to supporting individuals with diverse needs and backgrounds.
  • Strong organizational and time management skills, with the ability to prioritize tasks and adapt to changing priorities.
  • A current First Aid certificate is required.
  • A current National Crime Check is required.
  • A valid driver's license and access to a reliable vehicle are necessary.
