Aged Care Service Coordinator

The Clinical Aged Care Assessor role involves conducting comprehensive assessments to determine the needs of customers seeking home support services.
This is a permanent, full-time opportunity requiring travel in the Hervey Bay region to meet with customers and stakeholders.
The ideal candidate will possess exceptional customer service skills, strong communication abilities, and a deep understanding of aged care principles and practices.
Key qualifications include a tertiary qualification in a health-related discipline, current registration or eligibility to be part of a relevant professional association, and a minimum of one year demonstrated experience delivering services in aged care settings.

Main Responsibilities:
- Conduct comprehensive assessments using the Integrated Assessment Tool (IAT)
- Develop personalized support plans for customers
- Connect customers with relevant service providers
Requirements:

- Current unrestricted registration or eligibility to join a relevant professional association
- Minimum one year's experience delivering services in aged care settings
