
Care Coordinator

This is a full-time on-site role for a Case Manager located in Hervey Bay, QLD.
The successful candidate will be responsible for assessing client needs, developing personalized care plans, coordinating and monitoring service delivery, and providing ongoing support to clients.
This position requires strong assessment and care planning skills, excellent communication and interpersonal skills, experience in coordinating and monitoring service delivery, knowledge of relevant regulations and standards, organizational and time management skills, ability to work independently and as part of a team, and relevant tertiary qualifications in health or social sciences. Experience in the aged care or community services sector is beneficial. Proficiency in using relevant software and technologies is also required.

Requirements:
To be considered for this position, you will need to have:
Aged Care or Community Services Experience: While not essential, it is highly desirable to have experience working in the aged care or community services sector. This will enable you to understand the complexities of our clients' needs and develop effective care plans that meet their individual requirements.
Relevant Qualifications: You will need to hold relevant tertiary qualifications in health or social sciences. A degree in a related field such as nursing, social work, or psychology would be highly regarded.
Skills and Knowledge: You will need to possess strong assessment and care planning skills, excellent communication and interpersonal skills, experience in coordinating and monitoring service delivery, knowledge of relevant regulations and standards, organizational and time management skills, ability to work independently and as part of a team, and proficiency in using relevant software and technologies.
