About the Role

Cocoon SDA Care is a dynamic and innovative organisation seeking experienced Disability Support Workers to join our team. As a Disability Support Worker, you will play a vital role in supporting participants with complex care, personal care, and high physical needs in a Supported Independent Living (SIL) or SDA environment.

Key Responsibilities
  • Provide support to participants with complex care, personal care, and high physical needs in a SIL or SDA environment
  • Assist participants with daily living activities, including personal care, meal preparation, and household management
  • Develop and implement individualised support plans to meet the unique needs and goals of each participant
  • Work collaboratively with the multidisciplinary team to ensure seamless delivery of services and support
  • Maintain accurate and up-to-date records of participant progress and support provided
Requirements
  • Minimum of 2 years industry experience
  • Experience working with disability, mental health, high medical needs, aged care, individual support, and NDIS management
  • Clear and effective communication skills and ability to establish a professional relationship with participants and their families
  • Attention to detail and accuracy in record management
  • Flexibility to work over a 7-day roster and availability to work overnight shifts or attend training or meetings as required
  • Current First Aid, CPR, NDIS Orientation, COVID Infection Control, NDIS Workers Screening, Working with Children Check, and National Police Check
  • Current Open Driver's Licence
