About the Role

Cocoon SDA Care is a dynamic and innovative organisation seeking experienced Disability Support Workers to join our team. As a Disability Support Worker, you will play a vital role in supporting participants with complex care, personal care, and high physical needs. You will work in a Supported Independent Living (SIL) or SDA environment to help participants achieve their goals and live independently.

Key Responsibilities:

  • Provide support to participants with complex care, personal care, and high physical needs
  • Work in a Supported Independent Living (SIL) or SDA environment to help participants achieve their goals and live independently
  • Build relationships with participants and their families to understand their needs and preferences
  • Develop and implement individualised support plans to promote participant independence and well-being

Requirements:

  • Minimum of 2 years industry experience
  • Experience working with individuals with disabilities, mental health, or high medical needs
  • Clear and effective communication skills and ability to establish a professional relationship with participants and their families
  • Attention to detail and accuracy in record management
  • Flexibility to work over a 7-day roster and availability to work overnight shifts or attend training or meetings as required
  • Current First Aid, CPR, NDIS Orientation, COVID Infection Control, NDIS Workers Screening, Working with Children Check, National Police Check
  • Current Open Driver's Licence
