Job Title: Disability Support Worker

We are seeking a dedicated and compassionate Disability Support Worker to join our team at Help at Hand Support Services. As a Disability Support Worker, you will play a vital role in providing person-centered support to individuals with disabilities, enabling them to live independently and participate fully in their communities.

Key Responsibilities:
  • Provide daily living support, including personal care, meal preparation, and household management
  • Assist with mobility and transfers, as needed
  • Support individuals with disabilities to participate in community activities, such as shopping, social events, and recreational activities
  • Develop and implement individualized support plans, in collaboration with the individual and their support network
  • Monitor and report on the individual's progress, as required
Requirements:
  • Cert III in Disability or equivalent experience
  • Current Working with Children Check
  • First Aid/CPR certificates (current)
  • Full and clean Australian driver's license
  • Good written and verbal communication skills
  • Empathetic and proactive with a genuine interest to work in this field
Working Conditions:
  • Shift work, including morning, afternoon, and evening shifts
  • Rotating roster, including weekends and public holidays
  • Work in a variety of settings, including the individual's home, community centers, and other locations
